The form says that there can be 3 lines of engraving but the pictures show 5 in the small picture. Which is correct?

Are both boxes available - the one with the lighter color on top and the one below with the dark wood? How do I identify which I want? Is the stripes on the box inlaid wood or just some sort of decorative tape?
